さて、今日はしばらく書いていなかったHit-chanの初手術の記録です。
Today I want to write about my first surgery which I haven't talked about here for a while.
今日で手術から2ヶ月が経って、有難い事にすっかり普段に戻れているんですが、その分手術の記憶が薄れてきており、早くブログに書いていかなくちゃなぁと思っているところです。
Today on Feb. 5th, exactly 2 months passed from the surgery and thankfully I've been back to my healthy and normal life. The longer I've been used to a healthy life, the more I lose my memories about the surgery, so I'm thinking that I should finish writing my surgery record here before I forget everything about it.
ここまでの経緯はこんな感じ。
Here's the timeline so far.
10月30日 右下腹部痛開始
Oct.30th The pain in my right lower belly started
11月8日 1回目のナースライン（次の生理まで様子を見るようにアドバイスをもらう）
Nov. 8th The 1st nurse line call was made
11月16日 産婦人科の先生の診察（次の生理後にエコー検査をするようにアドバイスをもらう）
Nov. 16th App with an OBGYN doctor
11月28日 ER訪問

11月29日 かかりつけの産婦人科の先生の診察（今日の話はここです。）

ERで造影剤を使ったCTスキャンとエコー検査をしてもらった次の日、AndyっちがHit-chanかかりつけの産婦人科の先生との予約を11時半に取ってくれました。
On the next day after I did a contrast CT scan and ultrasound check at ER, Andy and I went to see my OBGYN.
普段なら予約ってすぐに取れなかったりするんですが、多分これまでの経過もあったので、電話をかけた当日に面談してもらえるようになったのだと思います。
I kind of remember that you wouldn't be able to make an appointment right away for most of the time, but my situation was probably counted as an urgent case and they let me see my doctor on the same day which Andy called them.
この日もめいさんはプリスクールがあったのですが、診察の予約時間とお迎え時間が被ってしまうので、Hit-chanは直接クリニックに向かい、Andyっちはめいをお迎えに行ってからクリニックで合流するというスケジュールになりました。
May-chan's at preschool as usual that day but the pick-up time was really close to my appointment time, so I headed to the clinic by myself and Andy picked up May-chan first and then met me at the clinic after.
楽だったのは、前日にERで受けた全ての検査結果が、かかりつけの先生がいるクリニックのシステムにも送られていた事。
What made our day easier was that the ER had already sent all of my information about the ER examination to my OBGYN.
面談の時には既に先生は全ての検査結果に目を通してくださっていたので、この日には特に追加の検査などもなく、この先の治療の選択肢を説明してもらって、治療方針を決めるだけでした。
So, my doctor had already reviewed all of my test results by the appointment, so all we needed to do was that she explained what treatment options that I could have and I chose which one I would want to do.
で、エコー検査の結果を見た先生が仰ったのは、
From my ultrasound result, she said
・エコー検査の結果を見たところ、出血性嚢胞と呼ばれるものに見える。その場合、自然に消えるケースもあり、人にもよるが大体8～12週間くらいで消える事が多い。ただ、嚢胞の影響で卵管が捻じれて強い痛みを起こす可能性もあり、その場合は手術が必要になる。
From the ultrasound images, the cysts look like what they call hemorrhagic cysts. In that case, they could be gone on their own and it differs how long it would take. Still, in most of the cases, it's fin 8-12 weeks. Although, there's also a possibility that the cysts will cause an ovarian torsion with a severe pain and when it happens, a removal surgery will be required.
・手術をする場合は腹腔鏡手術で、リスクは嚢胞を取り除く際に出血が多いと卵管や卵巣も一緒に除去する必要がある。
An surgery will be what's called laparoscopic surgery and there will be a risk that they would need to remove the ovarian tube or ovary when a lot of bleeding happens during the operation.
・また、可能性は低いとは思うが、子宮内膜腫（チョコレート嚢胞）という可能性もあり、その場合は手術が必要。
There is a small possibility that the cysts are what they call Endometriosis which will require a removal surgery.
でね、Hit-chanの選んだ治療方針は、「腹腔鏡手術をして嚢胞を取ってもらう」という事でした。
After the conversation with my doctor, I chose the option, "A removal surgery".
もし手術のリスクが「子宮を取らないといけないかもしれません。」という事なら、もう少し様子を見たかもしれませんが、もうかれこれ1ヶ月程痛みと付き合ってきて、本能的に「この痛みは自然には消えないな。」と思っていたのと、「最悪卵巣2個あるから、1個取っても良いか。」という気持ちで決めました。
（日本に凍結してある受精卵が2個あった事も大きな理由です。）
If the surgery had included a risk to remove my uterus, I couldn't have made the decision to do a surgery in such a short time, but there's almost no risk to lose my uterus and also the fact that I had been dealing with the ovarian pain for a month or so had made me believe, "This pain wouldn't go away on its own." with a hunch. Also, another part of me was thinking, "Yeah, I have 2 ovaries, so it'd be fine to get rid of one of them." too.
(Besides that, the fact that Andy and I still have 2 frozen embryos in a very good quality in Japan was a big help for me not to worry about losing one of my ovaries.)
そんな訳で、Hit-chanの決断を聞いた先生は、
So, my doctor respected my decision and told me,
「分かったわ。では、手術に向けての準備をするわね。スケジュールが合えば私が執刀するけど、合わない場合は私が信頼する先生にお願いするからね。」
"Okay then, I'll schedule an operation room for you. I'll operate the surgery if I'm available, but if I'm not, I want to pick a doctor who I trust."
との心強いお言葉。
そして、翌日にでもホームドクターに術前検診？の予約を取るようにとの事でした。
She also told me to make an appointment for a pre-operation check with my primary doctor on the next day.
で、この日は受付でホームドクターとの術前検診の予約を翌日に取ってもらい帰宅。
So, I made an appointment at the front desk and then left the clinic for the day.
最初、先生は「明日か明後日にでも手術室に空きがあったら、手術をしましょう。これからすぐに病院に確認して、手術室を押さえるようにするわね。」と仰ってくださっていたんですが、この師走の時期、病院も忙しくて手術室の空きが全くない事が判明。
On the appointment, my doctor was saying, "If they have an operation room available tomorrow or the day after, let's do it then. I'll call them and see if there's any room I can book." but later that day she found out that there's NO available operation room that week probably because it's in the end of the year which was the busiest season for hospitals.
先生との面談が火曜日だったのですが、次の週の月曜日に手術をする事になりました。
The appointment with my OBGYN was Tuesday and my very first surgery was scheduled on Monday on the next week.
アメリカって、保険の利用額が1月1日に更新される事が多いみたいで、「よし、年内に保険がカバーしてくれる分の治療をしよう。」と、年末駆け込み治療をする方も多いらしく、12月って病院も一番忙しい時期らしいんですねぇ。
※保険のプランに年間○○ドル以上払った場合、それ以降は全て医療費がカバーされるというのがあるんです。
In the United States, most of people have medical insurances which coverages are updated on Jan.1st, so a lot of people rush to do some medical treatment at the end of the year before their coverage gets reset, so I heard that December was the busiest season for hospitals.
何はともあれ、ERに駆け込んだお陰でかかりつけの先生が診断するのに必要な検査が一回で済んで、手術までのプロセスがスムーズに一気に進んでくれました。
Anyways, thanks to my decision to run to an ER, I could do all the required tests and exams at once there so that my OBGYN could examine and diagnose my case without any more additional tests, which made all the procedure to my surgery simple and quick.

さて、今日はしばらく書いていなかったHit-chanの初手術の記録です。
Today I want to write about my first surgery which I haven't talked about here for a while.
今日で手術から2ヶ月が経って、有難い事にすっかり普段に戻れているんですが、その分手術の記憶が薄れてきており、早くブログに書いていかなくちゃなぁと思っているところです。

ERで造影剤を使ったCTスキャンとエコー検査をしてもらった次の日、AndyっちがHit-chanかかりつけの産婦人科の先生との予約を11時半に取ってくれました。
